There are two ways that you may start receiving services from LifePath Hospice: Your doctor may write an order for LifePath Hospice to
evaluate and admit if appropriate, or the patient/family may call LifePath Hospice and ask us to call the patient’s doctor for the admit
order.
Please keep in mind that these admissions diagnoses are not all inclusive of the various hospice eligible disease diagnoses.
