Many patients feel that spiritual support is a very important part of their care. When personal chaplains or ministers are available,
LifePath can encourage their involvement. LifePath also has its own chaplains to facilitate spiritual support when needed.
LifePath Chaplains address the spiritual concerns of patients and their families within their individual belief systems, as well as
continuing to care for the family after the patient’s death to help them through this most difficult time.
Assesses the patient’s and the family’s spiritual needs
Develops patient’s plan of care to meet the identified needs
Provides direct counseling to the patient and family
Provides consultations to the clergy of the community
