You may call 1-800-327-3419 from 8:00 a.m. to 4:00 p.m.
Calls made after hours will be recorded and returned the next day.
As a patient of LifePath Hospice Care Services, you have the right to:
Receive a timely response from the hospice regarding your request for hospice service.
Receive information about LifePath Hospice Care Services policies and procedures and charges for services, including your eligibility for third party
reimbursement.
Choose your hospice.
Receive appropriate and professional quality hospice services regardless of race, creed, color, religion, sex, sexual preference, handicap, or age.
Be treated with courtesy and respect by all who provide hospice services to you.
Be free from mental abuse, physical abuse and/or neglect.
Receive proper identification by name and title of everyone who provides hospice services to you.
Receive all the information you need to enable you to give informed consent prior to your admission as a hospice patient.
Receive complete and current information concerning your diagnosis and prognosis in conformance with your physician’s legal responsibility to
disclose, in terms and language you can reasonably be expected to understand.
To have a hospice plan of care developed to meet your specific needs.
Participate in the development of your own hospice plan of care and receive assessments and updates as your plan changes.
Have privacy and confidentiality.
Review your clinical records upon request.
Receive information regarding anticipated transfer of your hospice care to another health care facility and/or termination of your hospice care.
Voice grievances and/or suggest changes in hospice services and/or staff without fear of reprisal.
Refuse treatment within the confines of the law and receive information concerning the consequences of declining treatment.
As a patient of LifePath Hospice Care Services, you have the responsibility to:
Give accurate and complete information concerning past illnesses, hospitalization, medication, allergies, and other pertinent items.
Assist in developing and maintaining a safe environment.
Assist in the development and update of your hospice plan of care.
Adhere to your developed/updated plan of care.
Request further information concerning anything you do not understand and share concerns and problems you may have with a hospice representative.
