The Social Worker is the patient’s principal contact for community resources. The patient and family are provided with direct support
and counseling.
LifePath Social Workers help patients and their families with a variety of psycho-social needs from dealing with the grief and loss of
a loved one to accessing additional support programs of various community agencies
Assesses the patient’s and family’s social, emotional and financial needs
Assists the patient and family with end of life planning
Develops patient’s plan of care to meet the identified needs
Provides patients and families with direct counseling or referrals to the appropriate community agencies
